We are a new team that has recently been developed to support young people who may be at risk of psychosis. The ARMS team is a preventative mental health team support young people aged 14-35 years old. It is a psychologically led service designed to prevent or delay the transition to psychosis.
This newly commissioned service is looking to recruit a wellbeing practitioner (qualified social worker/ mental health nurse/ OT). We are looking for someone with a passion for preventative mental health, who is dynamic, and driven to develop mental health services. You will be working in West Hertfordshire based at 3 Bowlers Green.
The ARMS Team will offer NICE recommended interventions, the wellbeing practitioner will be an integral part of the ARMS multi-disciplinary team. The teams will work in a mini team format and will include a wellbeing practitioner and STaR worker. The wellbeing practitioner will be leading on specialist assessments using the CAARMS (Comprehensive Assessment of At Risk Mental State) tool. Alongside providing specialised and tailored interventions and will help coordinate the individual's care.

Main duties of the job
The successful candidate will:
1. work closely with the CBT therapists and psychologists to formulate individual's care, to provide an effective multidisciplinary team approach
2. work alongside the PATH Team (Early Intervention in Psychosis Service) to ensure active, accessible care for the individuals
3. maintain links with other services in HPFT and the wider community.

You'll be offered a structured induction programme, giving you access to ARMS and psychosis learning and development opportunities to give you the confidence and skills to start delivering the ARMS pathway to our service users.

Job Summary:The post holder will work as a member of the multi-disciplinary Team,will be responsible for the clinical and operational day to day servicedelivery, providing a high quality care service to 16-35 year old at riskof developing psychosis.
The core functions are to participate in initial assessment of peopleusing a Comprehensive Assessment of At Risk Mental State Tool(CAARMS) to determine appropriateness for ARMS service. The teamwill work alongside the referring teams to identify alternative services asappropriate to meet the persons needs.
Once an individual has been accepted onto the ARMS pathway, thepost holder will be involved in a detailed assessment period exploringthe bio-psycho-social elements of the individual. The assessment periodwill also include engaging with carers and support network, alongsideinitial psychoeducation surrounding the ARMS ethos.
The post holder will be involved within psychology formulation to identifyclear goals and care planning. The ARMS team are using new, dynamictools including QPR and DIALOG questionnaires which will inform thejoint care planning.
The role will include understanding and delivering low intensitypsychological interventions to support individuals. Employment andvocational support, alongside signposting individuals to tailoredservices. The postholder will also engage with joint working withprofessionals, and work with individuals carers and support network tomanage risk and support the individuals mental health post holder will also be expected to deliver connected lives.Connected Lives is Hertfordshires whole service approach tocommunity practice, personalised enablement, and new models ofcommissioning for social care. Bringing together practitioners,commissioners, and providers, it will support the transformation of ourservice delivery. It is underpinned by the core concepts of the Care Act2014, ensuring community opportunities, citizenship, and personalisedoutcomes. Developed and launched by Hertfordshire County Council in2017, Hertfordshire Partnership University NHS Foundation Trust is nowintroducing Connected Lives as its model for social care practice acrossall adult services.
The model itself sets out standards, principles and approaches for staffdelivering services. It places more emphasis on prevention andenablement and supporting people to live the life they want. It can beapplied to every area of work, from frontline social work, to systems,leadership, and commissioning. As a member of the ARMS mini teamyou will be responsible for championing the model, and advocating itsprinciples to members of the multi-disciplinary team. You will hold a keyrole in providing social care assessments and commissioned packages of care, and will ensure that your practice ensures the service is meetingits statutory social care functions as described in the Care Act 2014.
