* LOCHVIEW – INPATIENT LEARNING DISABILITIES WARD
* DEMENTIA OUTREACH TEAM - Community

We are delighted to announce exciting opportunities available within the mental health services, in NHS Forth Valley. We are looking for applicants who have an interest in working within our mental health and learning disabilities teams.

Applications are invited from enthusiastic, caring and compassionate individuals. The post holder will work as part of a multidisciplinary team and should possess good interpersonal and communication skills (both written and verbal).

The role of the Clinical Support Worker is wide and varied and will support the delivery of care under the direction of registered mental health nurses. Please see the job description for further details of the role.

The successful candidate should be committed to the provision of high quality, patient focused care; have a flexible approach to service needs and a genuine interest in working with patients within a mental health or learning disability setting.

The posts are in the following locations

* Mental Health Unit, FVRH.
* Bellsdyke, Larbert
* Lochview, Larbert
* Dementia Outreach team – Areawide

There are 5 wards within the mental health unit, which specialise in providing assessment and treatment for people who are acutely unwell. These are busy clinical environments, and the role of the clinical support worker is pivotal in providing good care. As part of a multidisciplinary team, you will work together to provide a safe environment, observing and responding to patients changing presentation, using a therapeutic and empathetic approach. The overarching aim is to support people to return to their homes in the community, transition to other clinical wards for further rehabilitation or long-term care. You will be required to work shifts across 24 hours / 7 days per week

Bellsdyke, Larbert - We have posts available within both our low secure male and female wards. These wards specialise in providing therapeutic intervention for people requiring a low secure environment, therefore applicants need to be able maintain a safe and therapeutic environment. Care and treatment is centred around a rehabilitation model, promoting therapeutic meaningful occupation and activity, guided by individual risk assessment. Delivered by an MDT consisting of Nursing, Psychiatry, Activity co-ordinators, Occupational Therapy, Psychology, dietetics and Speech and Language. You will be required to work shifts across 24 hours / 7 days per week

Lochview, Larbert - Loch view is an in-patient service for adults aged 18 years and over with a learning disability diagnosis who require specialist or complex health assessment and treatment which cannot, at that time, be met in the community or by general health services. The unit's ethos of care and treatment focuses on Positive behaviour support plans and holistic individualised treatment plans. Our aims are to bring stability and intensive support to individuals, enabling them to return to their life in the community. We have the support of a full MDT including, Psychiatry, Psychology, Pharmacy, AHP's and a nursing team of Learning disability nurses, Mental health nurses and adult nurses with further support from our Health care support workers. You will be required to work shifts across 24 hours / 7 days per week

Dementia Outreach team – Areawide - The post holder will be based within the Dementia Outreach Team (DOT) who are currently based at Airth medical centre. The aim of the Dementia Outreach Team is to provide an intensive intervention to people with a confirmed diagnosis of dementia and their carers when they are in, or at risk of entering a crisis situation. As part of the Dementia Outreach Team the post holder’s role will include working collaboratively with health and social care professionals in primary, secondary and third sector community settings to meet the mental health needs of people living with dementia and promote patient centred care. Experience working in older adult is essential. Experience working within a community mental health setting would be advantageous. You will be required to work Mon – Fri, 9am-5pm. This post requires you to work and travel across all of Forth Valley, ability and willingness to travel throughout the area of service delivery is essential for this post.

Essential requirements

* Previous care experience. This can be in health, social care or private sector, or relevant qualification. This can range from health, social care or psychology qualification at SVQ level or equivalent.
* Applicants must be able to attend the interview in person at Forth Valley Royal Hospital, to enable ID checks to be completed.
