We are looking for an individual who is keen to engage with patients at our 13000 patient practice and supporting them in their care journey when needed. We are a dynamic and innovative practice and would welcome a Care Co‑ordinator to the practice who can really make this role their own. This role is to support the practice by co‑ordinating, integrating and delivering support to the practice patients whilst they are experiencing a medical episode of care (or multiple episodes simultaneously). You will ensure patients have good quality information to help them make informed choices about their care by liaising with a broad range of medical, social and third sector services.

You will liaise with secondary care clinics, supporting their requests for pre‑operative assessments conducted in primary care, this will include engaging with the patient organising appropriate appointments and informing correct clinicians of results.

You will work collaboratively with practice teams, including social prescribing link workers to meet the needs of the patient.


Main duties of the job

Patient care - Co‑ordinate, integrate and deliver support to the practice patients ensuring they have accurate and quality information choices about their care. Work with families and carers to improve health and wellbeing by navigating them to the appropriate services. Signpost patients to appropriate services and sources of funding and where appropriate to social prescriber.

Cancer care - Responsible to liaise‑by‑phone‑ with all patients that have been referred to the two week wait pathway within 10 days of referral to ensure all necessary clinics have been attended and patients are aware of next steps in their care. If a patient has a new cancer diagnosis, responsibility for sending a letter to the patients with support information. Contact with these patients at set intervals. Liaise with hospital departments, secretaries regarding patients. Delivering a quality service. Collaborative working within the practice and with external stakeholders. Able to work on their own as this role is the sole Care‑Co‑ordinator for the Practice, however working with the clinicians, management and administrators in the practice.


Job responsibilities

The Job description is per the overview of the role of Care‑Co‑ordinator. This role is for 32 hours per week which can be worked over 4 or 5 days.

Experience

* Experience of working in a GP Practice


Qualifications

* GCSE Grade C or above in Maths and English or equivalent qualification
* Demonstrate an appreciation of heath needs of the patient population within the practice.
* Communication skills both written and verbal
* Empathy with a variety of patients needs and conditions
* Positive attitude to work, colleagues and patients
* Tact and diplomacy
* Time managment
* Desire to help and put people first
* Ability to negotiate the best possible services for the patients
* Qualification in health or social care allied profession
* Knowledge of health and social care provision
* Knowledge of systm one clinical system

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.

£21,400 a year. Pay could be higher dependant on experience.
