Job role: Housing & Communities Connector Team: Community Investment Responsible to: Communities Manager Hours: 5 days a week (35 hours) Salary: £34,909.54 Contract: Permanent We’re looking for a Housing & Communities Connector to join our Community Investment team at CHS, helping our residents access the right support, build on their strengths, and sustain their tenancies. This is a hands-on role where you’ll work directly with residents, build relationships with partner services, and test new approaches to wellbeing and tenancy support. What you’ll do: Provide one-to-one support to residents, including home visits and advocacy Help residents navigate health, social care, and community services Co-produce personalised wellbeing and tenancy plans Link people to local activities, groups, and opportunities Work closely with CHS teams to address housing-related challenges Monitor progress and share learning to improve services Our approach is rooted in Asset Based Community Development (ABCD) and Human Learning Systems (HLS) - focusing on people’s strengths, building trust, and adapting support to real-life needs. About you: You’ll be a natural connector with the resilience, empathy, and organisational skills to support people with complex needs. You’ll be confident working both independently and as part of a team, and comfortable visiting residents in their homes. You’ll bring: Experience supporting vulnerable people or those facing social exclusion Strong communication and relationship-building skills Ability to assess needs and manage risks appropriately Knowledge of safeguarding good practice Basic IT skills (Word, Excel, email) A Level 3 qualification (or equivalent experience) in a relevant field A full UK driving licence and access to a car Why join CHS?
