An exciting opportunity has arisen for the recruitment of a Care Co‑ordinator within the Medicine Division. The post holder will be an integral member of the team responsible for managing and facilitating patient discharges from the medical wards. The shift pattern will be Monday‑Friday core office hours and, on a rotational basis, there will be the requirement to work weekends.

Education and qualifications
* Achievement through study to level 3 or above (NVQ, GCSE A‑C or equivalent, 1st Diploma, Apprenticeship)
* Level 2 Maths & English qualification or equivalent
Experience and knowledge
* Ability to work on own initiative and within a team
* Understands the importance of patient confidentiality
* Experience of working in a healthcare setting
* Knowledge of NHS Continuing Healthcare & Delayed Transfers of Care processes
* Experience in using patient information systems (PAS, eCare, Evolve LOSPT, Whiteboard, EPRO)
Skills and abilities
* Excel for Windows/10
* Accurate keyboard skills
* Able to demonstrate skills that ensure tasks are completed accurately and within a given timeframe
* Clear and precise communication skills
* Understanding Trust Policies with regards to requests for leave
* Able to work under pressure
* Able to meet deadlines
* Good interpersonal skills
* Able to demonstrate an understanding of issues relating to confidentiality
* Ability to work on own initiative or as part of a team
