We are supporting Leeds City Council to recruit a social worker to support their LD team. Contract will be for an initial 3-6 months with potential for extension subject to internal circumstance and performance.
Immediate start with ongoing contract available with competitive rates up to £35.00 per hour
Responsibilities include:

1. -Managing a complex cases
2. -To identify the needs of clients and mobilise available resources as appropriate.
3. -To provide professional assistance and guidance to colleagues within the directorate and other agencies as appropriate.
4. -To ensure that records are kept in accordance with the policies and procedures of the directorate and other agencies.
5. -Undertaking social work assessments under the Care Act and undertaking MCA and BI work.
6. -Undertaking holistic assessment of individuals and their carers' needs, arranging and/or delivering services to meet these needs and reassessing needs as appropriate.
7. -Implement care plans and care packages
Minimum Requirements:
8. -SWE Registration
9. -2 year post qualifying experience within UK
10. -Eligibility to work in the UK
