This is a time of significant change for both the Social Work Service in Dumfries & Galloway and the wider health and social care sector. This post offers opportunities to contribute to service delivery and development.
You will be an integral part of the Lower Annandale Home Team, working with colleagues from Adult Social Work, Community Nursing, Occupational Therapy, Physiotherapy, Public Health Improvement, and Patient Flow.
Your main responsibility will be to work with community members to assess their care and support needs and help develop and review outcome-based plans. This will involve collaboration with Carers, families, and colleagues in the Home Team, as well as links with the wider Health and Social Care Partnership and other agencies. Experience in health or social work settings is essential, with assessment and care management experience preferred. Knowledge of Self-Directed Support is advantageous.
You should possess excellent communication, assessment, analytical, and coordination skills, and be able to work independently, prioritizing tasks effectively. You should have an understanding of key themes relevant to health and social work, and an awareness of how aging, illness, and disability impact individuals. Practice must align with legislation, particularly the Social Care (Self Directed Support) (Scotland) Act 2013 and the Adult Support and Protection (Scotland) Act 2007. Commitment to evidence-based practice is essential, with support, supervision, and development opportunities provided.
A Dip/SW/CQSW/CSS qualification or equivalent is required, along with registration with the Scottish Social Services Council.
A current driving license is essential. If a disability prevents holding a license, alternative transport arrangements will be considered.
This role involves Regulated Work with Vulnerable Children and Protected Adults under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will need to join or update their PVG Scheme membership before employment is confirmed.
If you have lived outside the UK in the last five years, additional police checks from the relevant authorities will be required.
