Locations: North East - This role covers prisons and community sites all over the North East region so ability to travel is a key requirement of the role.
Hub locations - Middlesbrough, Durham, Darlington, Sunderland and Newcastle Prison locations - Northumberland, Durham, Holme House, Low Newton and Kirk Levington Grange

About the Role:
As part of this role, you will support a caseload of service users transitioning from custody to the community. You'll meet individuals in prison to begin planning their support in collaboration with their Support Worker, tailoring action plans to their specific needs. Post-release, you'll continue to guide them, connecting them with the full range of available services and support.
