Band 4 Falls Prevention Associate Practitioner – Clinical Practice Team
An exciting opportunity has arisen for a highly motivated and dynamic healthcare professional to join our Clinical Practice Team at UHD. We are seeking a Band 4 Falls Prevention Associate Practitioner to join our friendly and enthusiastic team.
The Clinical Practice Team is Trust‑wide and includes specialists in Falls Prevention, Tissue Viability, Clinical Digital Nursing and Professions, and Moving & Handling.
The Falls Team supports all UHD healthcare professionals, patients, and carers through the provision of education, advice, and service development to prevent falls and promote best practice when a fall has occurred. The team plays a key role in supporting compliance with national falls prevention guidance and contributing to the reduction of falls across the organisation.
This Band 4 role will work under the direction of senior clinicians within the Clinical Practice Team, supporting falls prevention projects, education delivery, audit activity.
Rota: Monday to Friday, 8:00–16:00 or 9:00–17:00, to ensure service coverage
Location: Royal Bournemouth and Poole Hospital – the post‑holder will work across sites

Main duties of the job
Your role will support the delivery of the Trust Falls Prevention Programme and contribute to monitoring the effectiveness of the Trust Falls Strategy, working under the supervision of senior clinicians to support compliance with national and local falls prevention guidance.
The role will involve providing advice and practical support to clinical areas, assisting with the coordination of After Action Reviews of falls incidents in line with Trust processes.
In addition, the post‑holder will contribute to Trust‑wide education, audit, and quality‑improvement activities to support awareness and best practice in falls prevention.
Strong IT skills are required to support data collection, reporting, and learning dissemination, alongside an ability to engage positively with staff around patient safety initiatives.

Person specification
* Achieved NVQ level 4 or equivalent qualification
* Experience working in a healthcare or NHS setting, preferably within an acute Trust environment
Knowledge
* Knowledge of falls prevention and actions to take following a fall
* Recent experience as a Healthcare Support Worker or Therapy Assistant involved in direct patient care
