Job overview

To provide an integrated heart failure (HF) service for residents within Cardiff and Vale working closely with primary care.

The role of the band 6/7 Heart Failure Clinical Nurse Specialist is to provide specialist advice and guidance to clinical teams, patients and carers, both in the outpatient and inpatient setting. Their role includes; direct clinical assessment, medication prescription and detailed specialist guidance for the MDT caring for a patient with confirmed heart failure. Essential to the role is excellent written and communication skills and the ability to work both autonomously and within the broader heart failure MDT.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.


Main duties of the job

An exciting opportunity for an enthusiastic Band 6/7 Heart Failure Specialist Nurses to join our expanding team at CVUHB. We are an established, integrated heart failure service covering 2 acute hospital sites one being a tertiary centre. The posts will support the implementation of community heart failure clinics for our local population, as well as supporting the current inpatient and outpatient services.


We look forward to recruiting an individual who provides a high standard of evidence-based care with experience of managing heart failure patients and who is committed to continually improving the service provided for heart failure patients.

The individual will work collaboratively with the MDT to ensure that patients receive seamless care in hospital, at home and in the outpatient setting. Ideally, we are seeking a nurse with experience in working autonomously with HF patients. All applicants must reflect a good understanding of the specialty and role as well as a strong commitment towards improving care within heart failure. However, if you are interested in starting a career in HF we are able to offer a band 6 training position with the aim of successfully achieving competence to move to band 7 once clinical competencies are achieved.
