Job Overview
The CRT Pharmacy team was established in 2019, providing services to Newport, Caerphilly, and Torfaen. Due to the ongoing success of the service, we now have Pharmacy Teams across all five boroughs and are looking to recruit a Pharmacy Assistant to join our established team in Caerphilly.

Main Duties of the Job
The team currently provides pharmacy services to:
* Frailty Consultant-led ‘hospital at home’ patients
* Therapies/Social Care-led Reablement patients
* Nurse-led home parenteral service patients (including IV antimicrobials)
* Community Falls patients
We also handle referrals from acute ward pharmacy staff to support frail and vulnerable patients in medication management post-discharge. Our responsibilities include participating in MDT meetings, home visits, virtual ward rounds, and data collection and analysis to contribute to patient care. Each day presents new challenges, opportunities to learn, job satisfaction, and a chance to develop the service further.
The CRT Pharmacy Teams are now successfully integrated with community hospital rehabilitation wards in South Monmouthshire and Torfaen.
This unique role involves working across the primary and secondary care interface, fostering excellent relationships with community, practice-based, and hospital colleagues.

* Ability to undertake simple arithmetic calculations without a calculator
* Good verbal and written communication skills
* Computer literacy

* Ability to speak Welsh
* Basic knowledge of NHS functions
Qualifications
Essential criteria
* Good standard of secondary education
* Vocational/NVQ Level 3 or equivalent training to the General Pharmaceutical Council's requirements or willingness to undertake
Desirable criteria
* GCSE at C grade or above in Maths, English, and Science or equivalent qualifications
* Current knowledge of hospital pharmacy practice
