South & WestHerefordshire Primary Care Network is looking for a Care Coordinator tojoin our Health & Wellbeing Team. Theposition will work with our Frailty and Neighbourhood Health teams to supportthe PCN in offering access to proactive and reactive personalised care andsupport, with a particular focus on supporting those patients at highest riskof suffering from health inequalities.

The PCN iscomprised of six GP practices centred around the market town ofRoss-on-Wye: Alton Street Surgery,Pendeen Surgery, Fownhope Surgery, Much Birch Surgery, Golden Valley Practiceand The Surgery Kingstone. The rolewill be hosted by the Herefordshire GP Federation, Taurus Healthcare, as theemployer.

* This is a Fixed Term position for 18 months *

*Interviews are expected to be held on Thursday 23rd October 2025*


Main duties of the job

* To work closely with the PCN and practice teamsto identify and work with a cohort of people to support their personalisedcare requirements, using any available decision support tools.
* To act as the primary point of liaison for thePCNs multi-disciplinary team meetings, capturing action steps at allmeetings, and ensuring that all agreed activity is completed.
* To contact patients identified for outreach toarrange for appointments and to promote healthy lifestyle choices, such assmoking cessation and health checks.
* To support the members of the Frailty andNeighbourhood Health teams with outreach to identified cohorts of patients.

Job responsibilities

TheNetwork Care Coordinator (NCC) role acts as the central care coordination rolewithin the S&W PCN. Supporting the networks Health & Wellbeing Teamand the practices in executing on their contractual requirements, the role willact as the central point of contact for outreach to a range of patientsidentified as requiring clinical or wellbeing intervention. These will include frail patients, patients atrisk of Health Inequalities and patients with predeterminants of cardiovasculardisease.

South& West Herefordshire PCN is made up of six practices centred around themarket town of Ross-on-Wye: Alton StreetSurgery, Pendeen Surgery, Fownhope Surgery, Much Birch Surgery, Golden ValleyPractice and The Surgery Kingstone. Thenetwork covers a wide geographic area offering traditional General Practicesupport to our patients centred around the communities they live in. The role will be hosted by the HerefordshireGP Federation, Taurus Healthcare, as the employer.

TheNCCs role requires them to be able to work with, and understand the roles of,a variety of different people working across the PCN including doctors, nurses,healthcare assistants, social prescribing link workers, physiotherapists,health and wellbeing coaches and pharmacists.

They will identify and work with individuals in need of proactivesupport with the aim of providing advocacy, encouraging independence, a healthylifestyle, mental wellbeing, and social connectivity. They may be given a caseload ofidentified patients and be required to ensure that their changing needs areaddressed by taking into account local priorities, health inequalities and/orpopulation health management risk stratification.

Thesuccessful candidate will be a caring, dedicated, reliable, person-focussed andenjoy working with a wide range of people. They will have good written andverbal communication skills and strong organisational and time managementskills. They will be highly motivated and proactive with a flexible attitude,keen to work and learn as part of a team and committed to providing people,their families and carers with high quality support.

Person Specification


Experience

* Working knowledge of Microsoft office Word, Excel, Outlook and PowerPoint and using video calling software e.g. Microsoft teams.
* Experience of working in a health or social care setting
* Experience of providing one to one and/or group support.
* Completed a two day PCI accredited care co-ordination training course or be willing to complete one.
* Experience of using IT systems such as EMIS.
* Experience of partnership/collaborative working and of building relationships across a variety of organisations.
* Knowledge of the core concepts and principles of personalised care, shared decision making, patient activation, health behaviour change, self-efficacy, motivation and assets-based approaches.
* Experience of minute-taking within an MDT environment.


Skills

* Active and empathetic listening, provide personalised support in a non-judgemental way.
* Ability to support people in a way that inspires trust and confidence, motivating others to reach their potential.
* Ability to provide a culturally sensitive service supporting people from all backgrounds and communities, respecting lifestyles and diversity.

* Required to travel to meetings and work from other locations as required.


Qualifications

* GCSE English or equivalent level.
* GCSE Mathematics or equivalent level.
* ECDL or equivalent level of keyboard/IT skills.

£24,506.79 to £29,519.54 a yearpro rata depending on experience
