Registered Nurse

You will be responsible for the assessment, planning, implementation and evaluation of care forService Users within the WICU community ward. You will provide high quality care that meets theneeds of Service Users who require an inpatient bed to either avoid hospital admission, forrehabilitation, or further assessment of their nursing needs which can include managing complicatedlong-term conditions, recognition and response to deterioration and care towards the end of life.

You will also contribute to the development of a positive working environment, demonstratingSCARF principals and be committed to the development of yourself and others.

Main duties of the job

* To provide skilled, effective and evidence-based nursing care to service users.
* To provide care and attention to the needs of vulnerable adults. Including assessment, planning, implementation and evaluation of care.
* To have a good understanding of what has led to a person requiring our service, what needs to be done to get them home and taking a pro-active approach to discharge planning.
* To ensure that all care plans in the ward are completed, up to date and evaluated regularly.
* To liaise with, and refer to, all those involved in the delivery of care to ensure adequate care and support for service users.
* To undertake assessments for deteriorating patient, continence, management of falls assessment and prevention, pressure ulcer prevention and management, medicines management.
* To undertake nutritional assessments and advise and support people with their nutritional needs e.g. thickened fluids and special diets
* To work with older people with intermediate care needs providing a rehabilitative approach to care, which promotes maximum independence.
* To undertake Mental Capacity Assessments within your scope of practice, recording and acting on these according to Trust guidance.

* Deliver exceptional nursing care to patients with complex long-term conditions
* Collaborate with patients, families, and a multi-disciplinary team to enable people to leave hospital safely
* Conduct key assessments including patient deterioration, mental capacity and continuing healthcare checklists
* Support and teach colleagues, students, patients, and carers
* Work within local and national guidelines to ensure safe, evidence based and dignified care

Job responsibilities

* To provide skilled, effective and evidence-based nursing care to service users.
* To promote the participation of service users and relatives in assessments and care planning.
* To provide care and attention to the needs of vulnerable adults. Including assessment, planning,implementation and evaluation of care.
* To have a good understanding of what has led to a person requiring our service, what needs to bedone to get them home and taking a pro-active approach towards discharge planning according tolocal policy.
* To ensure that all care plans in the ward are completed, up to date and evaluated regularly.
* To participate in weekly multi-disciplinary clinical reviews.
* To liaise with, and refer to, all those involved in the delivery of care to ensure adequate care andsupport for service users.
* To identify and record all incidents and concerns and report back to the Ward Manager.
* To ensure the safe handling of body fluids and contaminated sharps.
* To undertake assessments such as; assessment of the deteriorating patient, continenceassessment and management of falls assessment and prevention, pressure ulcer prevention andmanagement, medicines management.
* To undertake MUST nutritional assessments and advise and support people with theirnutritional needs e.g. thickened fluids and special diets under the guidance of the specialistdietitian.
* To provide a teaching role for colleagues, students, service users and carers.
* To work in accordance with the Nursing and Midwifery Council, Trust policies and guidelines tomaintain accurate and detailed recorded records and recognise legal and ethical issues.
* To work with older people with intermediate care needs providing a rehabilitative approach tocare, which promotes maximum independence.
* To undertake Mental Capacity Assessments within your scope of practice, recording and acting onthese according to Trust guidance.
* To be alert to the needs of adults at risk, identifying risk of possible harm and taking appropriate safeguarding action as required.
* To maintain accurate and contemporaneous records using the Electronic Patient Record system andElectronic Prescribing on Systmone


Person Specification


Other

* Ability to work over a 247 service


Qualifications

* Is an RN (adult) on the current UK NMC register
* Evidences ongoing CPD
* Has a BSc Nursing (adult) Degree or equivalent qualification


Knowledge

* Demonstrates an understating of Intermediate Care and Rehabilitation
* Is able to demonstrate the ability to assess, plan, deliver and evaluate care to meet the needs of the service users
* Shows effective communication at all times
* Understands and refers to relevant local and national policy and guidance relevant to the care of the older person, and intermediate care
* Has sound IT skills and can work across multiple systems


Experience

* Has experience of Interprofessional working
* Has experience of working within an intermediate care setting, or a ward setting
