About the role
As a Community Care Officer (CCO) within our newly established Mental Health Social Work Team, you will play a vital role in supporting adults with care and support needs to live fulfilling, independent lives within their communities. This is an opportunity to be part of a developing service, shaping how we respond to mental health needs through a person‑centred, strengths‑based approach.
Key responsibilities include:
* Managing a diverse caseload of non‑complex and complex mental health cases
* Completing strengths‑based Adult Needs Assessments and Reviews in line with the Care Act
* Building therapeutic relationships with individuals, families and carers
* Supporting people to define and work toward meaningful, recovery‑focused outcomes
* Promoting independence, resilience and community inclusion
* Identifying and addressing barriers that impact wellbeing and progression
* Working in partnership with health, housing, and voluntary sector colleagues
* Responding flexibly to service demands, including unplanned visits
* Managing hybrid working: home and office based collaboration as required
* Traveling efficiently to meet service needs

About the team
The Mental Health Social Work Team at Lincolnshire County Council brings together Social Workers and Social Work Assistants across three localities, creating a supportive and collaborative environment where every contribution is valued.
