Job overview Are you a passionate and motivated Physiotherapist or Occupational Therapist looking for a role where your work truly makes a difference? Join Sirona as a Senior Community Therapist and play a key role in supporting people to live independently and well at home. This is an exciting opportunity for a clinician committed to high-quality, community-based rehabilitation and person-centred care. You will work with individuals in their own homes to improve quality of life, promote independence and support long-term self-management. We have part-time and full-time roles available in: • South Bristol • Inner City & East Bristol (ICE) • North & West Bristol You will be supported to: • Develop specialist community therapy skills • Strengthen multidisciplinary working • Build confidence supervising and supporting colleagues • Grow leadership skills within a supportive environment The Sirona Integrated Network Teams (INT) are innovative services delivering active rehabilitation in community settings. We support: • Hospital discharge and admission avoidance • Rehabilitation following illness or injury • Long-term condition management Our focus is maximising functional potential and enabling people to remain safely and confidently at home. You will join a friendly multidisciplinary team of Therapists, Assistant Practitioners, Nurses, Advanced Clinical Practitioners, Support Workers and Administrators. Advert Initial holistic assessment, implementation and evaluation of programmes of care for patients who are acutely and chronically unwell; often with complex needs. Use clinical reasoning skills to assess, plan, implement and evaluate, patient- centred intervention programmes using an evidence-based practice approach, promoting high quality and harm free care Responsible for proactive service and own caseload management Responsible for allocation of work and efficient service/team coordination Develop and maintain strong, communicative working relationships to prevent avoidable admissions and facilitate timely discharges. Collaborate and negotiate with service users, carers and family in identifying achievable goals as part of the overall care plan to maximise quality of life and independence. Facilitate appropriate referrals to other services based on your assessment Assess and arrange the provision of equipment within scope of practice; reporting any issues Take responsibility for the development of other members of the INT including peers, junior staff, and students Maintain accurate, informative and legal records Actively participate in training; this may involve assisting in the delivery Identify risks and act accordingly to minimise risk to self, colleagues and service users Working for our organisation Ever dreamt about a career that sparks excitement and fosters your professional growth?
