We have an exciting opportunity for a Healthcare Support Worker to become part of our dedicated team in Devizes, working 20 hours and 30 minutes per week. At HCRG, we're passionate about expanding and enhancing our community services, aligning with the NHS Long Term Plan to transform 'out of hospital' care. We're strengthening our existing community teams by increasing response capacity and introducing a 2-hour crisis response element, ensuring high-quality care around the clock. Now, we're extending our nursing services 24/7 across Wiltshire! Our overnight nursing service will provide essential care when patients need it most. Shift times: 20:15 - 07:45. You'll work alongside experienced nurses, delivering effective interventions in homes and community settings, helping prevent hospital admissions, and responding swiftly to urgent referrals.
Responsibilities:
1. To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
2. To undertake the daily care and support of delegated patients.
3. Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
4. Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patient receive the appropriate care in the most appropriate setting.
5. Responsibility for the day-to-day care of patients including monitoring.
6. Support the taking of medication under the authorisation of the qualified practitioner.
7. To have a good knowledge of infection control, to reduce risk of health care acquired infections.
8. To enable patients to reach their optimum level of independence.
9. Support terminally ill patients and their carers/relatives.
10. To communicate with individuals about routine and daily activities, this may include overcoming difficulties (e.g. hearing/language).
11. To ensure the needs and wishes of the individual are documented.
12. To work within a multi-professional team.
13. To undertake specific tasks as delegated.
14. To assist registered and junior staff, referring to registered staff when necessary.
15. To practice the principles and philosophy of individualised patient care in accordance with policy, procedures and regulatory body's guidelines.
16. To obtain consent for any treatment undertaken.

If not already completed, a Care Certificate must be completed within 3 months of employment. A UK driving licence and appropriate business vehicle insurance are required for this role.

* NVQ Level 3 in health care - or willingness to achieve.
* Experience of working under distanced supervision.

* Physical skills to manually handle patients and use appropriate lifting aids.

* Current full driving license and use of a car during working hours.

* Food Hygiene certificate or willingness to undertake.
