Community Support Worker - HomeFirst Restormel

The role is similar to a Health Care Assistant in that you will be delivering great care and supporting reablement for people, mainly in their own homes.

You will be working as part of the HomeFirst Restormel team, covering Newquay, St Austell, Padstow, and surrounding areas. Travel costs for journeys during work time are supported and pool cars are also available for work use although availability cannot be guaranteed. Due to the rural location of Cornwall, public transport will not be sufficient to get to patient visits. Ability to get to visits independently is essential for this role, usually you would drive and have access to your own vehicle to achieve this.

You will need to demonstrate good communication skills and a positive approach to people who may have communication difficulties or who have challenging health needs.

You will be required to work a rota that covers 7 days a week from 08:00 to 20:00 hours. Flexibility within this rota, and geographical location, will be important.

This is a lone worker post.


Main duties of the job

* The main purpose for this post is to provide a versatile and responsive service to ensure that opportunities for patients' reablement are optimised within the community setting. Assist the patient to achieve their reablement goals using a range of specific activities, primarily on an individual basis, but also in groups, without direct supervision. This may include activities for developing independence in daily living skills, e.g., supporting with personal care and promoting a return to normal function and activity as appropriate, support and encourage the appropriate use of aids/equipment and reinforce self-care wherever possible. The Reablement workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital. The post holder will be expected to deliver packages of care to patients in their own home; this may also include Residential homes and nursing homes. The post holder will be expected to be flexible in working patterns to meet the needs of the service and will be rostered to provide the required service. Person Specification


Education, Qualifications and Experience

* NVQ Level 3 qualification in care or equivalent experience
* Evidence of experience in care setting
* Additional relevant training in Healthcare


Skills and Aptitude

* Flexible in attitude and willing to support in other teams when required
* Ability to plan own workload and organise required activities


Personal Qualities

* The ability to deal calmly with stress and distressing situations
