MDT Cancer Pathway Co-ordinator

An exciting opportunity has opened for a motivated and enthusiastic individual to join the Cancer Services team at Warwick Hospital as part of the MDT Team. As the Patient Pathway Coordinator, the successful candidate will play a pivotal role in achieving the trust's cancer targets by ensuring that each patient's journey is managed efficiently and progresses in a seamless and timely manner.The role involves close collaboration with various departments across the trust, including clinicians, general managers, deputy managers, nurses, secretaries, and other administrative staff.

The position offers an agile working arrangement, combining both office-based and remote work. While some days may be spent working from home, the number of remote working days will depend on the needs of the service. The successful candidate will be expected to work from the office as required.

The ideal candidate will have prior experience working within the NHS, along with administrative experience and a solid understanding of cancer plans, standards, and targets.


Main duties of the job

As the Patient Pathway Coordinator, the post holder will play a critical role in the management and coordination of cancer patient care within the trust. A primary responsibility will be to track the progress of cancer patients in strict alignment with national cancer targets, ensuring that each patient's journey is monitored and managed effectively. This involves the careful collection and entry of data related to both suspected and diagnosed cancer patients into the relevant databases, guaranteeing that all information is accurate and up-to-date. In addition to patient tracking, the role extends to providing comprehensive administrative support to the MDT (Multidisciplinary Team) Facilitators.

This support is integral to the smooth operation of the MDT meetings and the overall management of cancer pathways within the trust. The post holder will also play a key role in contributing to the data collection required for the National Cancer Audits. By ensuring the accurate and timely collection of audit data, the post holder will contribute to the trust's ability to meet national benchmarks and improve patient outcomes.

This position is vital to maintaining the high-quality information that underpins the cancer pathway, ensuring that patients receive timely and effective care.

Job responsibilities


Knowledge, Skills and Experience

Using hospital systems including Somerset, Lorenzo, Evolve, Soliton, ICE, Medical Records, MOSAIQ and ICR (external systems) to monitor and record every event in the Patient Pathway for all tumour sites.

To retrieve information needed to maintain a tracking process on every two-week referral, consultant upgrade and screening referrals from the in-house and external systems.

Working with Cancer Patient Pathway Facilitators ensuring that pathways reflect the requirements agreed for each tumour site Trust timeline.

Weekly updates to the Cancer Data & Performance Manager regarding the progress of individual patients in the diagnostic stage, escalating issues and delays in the patients pathway as identified by the diagnostic pathway report.

Gain an understanding of the medical terminology of diagnosis and treatments for cancer in order to fully understand the patients pathway for the whole cancer pathway.

Provide administrative support as required to all members of the team including MDT meeting preparation and out coming agreed actions.

Ensure the provision of a high quality service. To ensure that all Trust standards and targets are maintained and monitored to improve the quality of care to all who come into contact with services provided by South Warwickshire NHS Foundation Trust.

Undertake such other tasks as required by the Cancer Data & Performance Manager in accordance with the grade and nature of the post.


Measurable Result Areas

Input data into Somerset to ensure uploads are representative of the Trust Cancer waiting Times position in line with national targets and standards for all tumour sites.

Ensure the cancer database is updated in a timely and accurate manner to assist clinical teams to achieve cancer waiting time targets.

Maintain filing systems and updates and retrieve information as necessary.

Work in electronic document management systems and input information into live systems.

Ensure data collection is undertaken in Accordance with Data Protection Act.

Participate in ongoing service improvement and data quality monitoring, collation and formatting of data and ensure data is accurate, timely, validated, complete, consistent, and unique.

Participate in the maintenance and upgrade process of the Somerset Cancer Registry Database.


Communications and Working Relationships

Maintain close communication with the Cancer Patient Pathway Facilitators; updating them on a regular basis on work progress and alerting them of any problems that may result in a delay in the patients treatment.

Liaise with all relevant departments; administrative, clerical, medical and nursing to ensure outpatient and inpatient appointments are booked in a timely manner. Support the Cancer Patient Pathway Facilitators to send and receive tertiary referrals and investigation / treatments results for patients from other Provider organisations in line with the local agreements and national targets.

Ensure effective communication with nursing staff, medical teams and other co-ordinators to ensure the organisation and coordination of timely management of all patients on a cancer pathway.

Communicate with medical, clerical and management potentially sensitive and contentious information in the progression of the patients pathway.

Work with clinical teams to assist in the collecting and recording data items for National Cancer Clinical datasets e.g. Cancer Waiting Times, COSD, Clinical Audit and SACT data as required.

Feedback any comments or suggestions received from service users to all members of the multi-disciplinary team and help facilitate any changes made to improve the patient pathway.


Person Specification


Qualifications

* Good standard of general education (4 GCSEs or equivalent), including Maths and English language
* RSA Level 3/ HNC or equivalent
* Computer literate with evidence of Hospital systems
* Knowledge of Microsoft Software


Experience

* Previous knowledge of NHS
* Able to maintain accurate and timely records
* Experience of team working
* Experience of tracking and recording Patient pathways against National Performance measures ie RTT, CWT, Diagnostic targets
* Experience of patient pathway coordination


Skills

* Ability to motivate self
* Able to work unsupervised
* Able to work to deadlines & prioritise
* Have the confidence to liaise with all levels of staff across the Trust
