Responsibilities
* Deliver high-quality, person-centred care to individuals’ homes and within their communities.
* Support individuals in maintaining or developing new skills and personal interests.
* Adapt care and support plans to meet the diverse needs of each individual, ensuring their comfort and safety.
* Engage in proactive communication with families and care teams to ensure effective care delivery.
* Support individuals in temporary residential placements or their own homes.
* Perform household chores such as laundry, cleaning and food shopping.
* Manage and support individuals with budgeting and financial management.
* Assist with all aspects of personal care, including hygiene, dressing and feeding.
* Plan, participate in and budget for community activities.
* Maintain accurate records and clear communication with the brand team and other healthcare professionals.
Qualifications and Experience
* Preferred NVQ Level 3 in Health and Social Care (Level 2 acceptable).
* Minimum of 6 months of experience in similar healthcare settings and experience in support with complex needs, behaviours that challenge and autism.
* Business Car Insurance would be advantageous.
* Full UK driving licence would be preferred.
