Overview
South Warwickshire University NHS Foundation Trust is recruiting four Specialist District Nurses to join our Out of Hospital Care Collaborative (OOHCC). These roles will lead care in four distinct localities, including Rugby, one of the fastest growing towns in Warwickshire.
Responsibilities
* Assess and manage complex health needs within your locality.
* Lead the development, implementation, and evaluation of care programmes.
* Provide clinical leadership for all patients within the community nursing caseload through proactive management.
* Work collaboratively with a multidisciplinary team to adopt a whole‑systems approach to patient care.
* Ensure that all members of the nursing team deliver evidence‑based clinical care.
* Clinically supervise and develop staff, including performance reviews, clinical supervision, recruitment, and selection.
Qualifications & Requirements
* Registered Nurse with substantial community experience managing patients with complex needs.
* Advanced clinical assessment skills.
* Completion of the DNSPQ qualification.
* Independent prescriber.
