Community Mental Health Lead Practitioner

NHS AfC: Band 6 • Grade: Band 6 • Contract: Permanent • Hours: Full time – 37.5 hours per week • Site: Bognor Neighbourhood Mental Health Team, Bognor Regis • Reference: 354‑AC‑21206 • Salary: £38,682–£46,580 per annum (pro rata for part time).

Job Overview

Bognor Neighbourhood Mental Health Team – Lead Practitioner – qualified nurse, occupational therapist or social worker with a keen interest in working in a supportive multidisciplinary setting with patients experiencing mental health difficulties.

Usual hours: Monday to Friday 9 am – 5 pm. Candidates must be able to travel independently across the locality and have access to a suitable vehicle for business purposes.

Post offers the Trust’s £2,000 Golden Hello or Relocation Incentive for Band 6 nurses and practitioners (pro rata and subject to terms and conditions).


Main Duties of the Job

* Maintain patient safety in collaboration with patients, carers and the team.
* Recognise potential for or signs of patient harm, abuse or neglect and take all reasonable steps to protect the patient.
* Provide clinical advice on complex issues to the clinical team and staff from other disciplines.
* Undertake urgent and priority assessments with risk assessments.
* Manage decision‑making and coordination in crisis situations.
* Provide leadership when the Care Coordinator / Lead Professional is absent and urgent responses are required.
* Support the team where Intensive Home‑Based Treatment is required.
* Liaise with other services such as Police and A&E as required.
* Participate in setting quality standards and delivering a consistent approach to care.
* Support the team in delivering interventions in accordance with the agreed care plan.
* Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
* Provide specialist assessments, e.g. Mental Capacity Act, Deprivation of Liberty Safeguards, safeguarding adults and making safeguarding personal.
* Maintain professional standards of care at all times.

Detailed Job Description And Main Responsibilities

Excellent communication skills are essential for liaising with all stakeholders including service users, carers, third sector providers and external agencies. We encourage applicants with lived experience and desire to develop into a community mental health nurse role. We provide induction, ongoing supervision and training.


Person specification


Qualifications

* Registered Nurse or other health professional with current registration appropriate to the role.
* Completion of Mentorship Course/ENB or professional equivalent.
* Evidence of continuous professional development.


Knowledge/Experience

* Significant post‑qualifying experience in mental health or neurodevelopmental care.
* Experience working with people with complex health problems.
* Experience supervising staff.
* Experience providing a range of clinical interventions to people in the designated care group.
* Experience working consultatively with professionals.
* Experience conducting clinical and risk assessments.
* Experience working within and across statutory and non‑statutory teams and agencies.
* Experience working in the community.
* Understanding of clinical governance.
* Working knowledge of the CPA process, safeguarding adults and children, and related policies.
