Job Title: Social Worker - Hospital Discharge Location: Islington Pay Rate: £33.73 per hour Job Description: Liquid Personnel is recruiting a Social Worker for its client’s Hospital Discharge Team in Islington. We’re seeking an experienced Adult Social Worker to join a fast-paced Hospital Discharge Team, supporting safe and timely discharges from both acute and community hospitals. You will work closely with health professionals, patients, and families to ensure care and support needs are met - promoting independence and preventing unnecessary delays. What will your responsibilities be?
