Adults Social Worker - Transitions Team Halton, Cheshire Up to £35 per hour Temporary Contract Connect2SocialWork are currently recruiting for an experienced Adults Social Worker to join a well-established Transitions Team in Halton. This is a fantastic opportunity to support young people aged 14 as they move from children's to adult services, ensuring smooth, person-centred transitions in line with Care Act eligibility criteria. Key Responsibilities Screen incoming referrals within the Transitions Team Complete Care Act assessments in line with statutory requirements Develop and implement person-centred support plans Work collaboratively with young people, families, and multi-agency professionals Support effective transition planning into adult services Maintain accurate, timely, and compliant case records Requirements Qualified Social Worker with relevant professional registration Strong experience completing Care Act assessments Excellent assessment, analytical, and decision-making skills Proven ability to work effectively with families and multi-disciplinary teams Desirable Experience working with Looked After Children (LAC) Background in Transitions or Disability teams Essential Knowledge Strong understanding of how disability impacts family life Ability to engage sensitively with young people and families during key transition stages Why Work with Connect2Socialwork?
