Are you passionate about helping others? We are seeking an enthusiastic Community Care Officer to join our Broxtowe Community Social Work Team. In this role, you will support individuals and their families to live fulfilling lives, regardless of age or disability.
Key Responsibilities
* Identify Goals: Understand individuals' aspirations and what a good life means to them.
* Coordinate Support: Initiate and coordinate outcomes, including reablement and enablement opportunities.
* Promote Independence: Use strength-based approaches to encourage choice, control, and self‑directed support.
* Collaborate: Work with health colleagues and other professionals to provide comprehensive support.
* Flexibility: Be prepared to work across different districts as needed.
Required Skills
* Relevant qualification in social care or a related field.
* Experience in adult social care or community support.
* Knowledge of strength-based approaches and person‑centred planning.

* Empathy: A genuine desire to understand and support others.
* Communication: Strong verbal and written skills.
* Teamwork: Ability to collaborate effectively.
