The Role
We are looking for an enthusiastic and motivated Social Worker to join our Community Learning Disability Team. Ideally, you will have experience working with people with learning disabilities, but we also welcome newly qualified applicants and offer strong support through ASYE and ongoing professional development.
You will be part of a high-performing, supportive, and integrated multi-disciplinary team that values collaboration, learning, and delivering quality outcomes for Bolton residents.
Your role will involve assessing needs under the Care Act, developing asset-based support plans, completing carers assessments, capacity assessments, Best Interest decisions, and Continuing Health Care assessments. You will work closely with other professionals and agencies to ensure effective service delivery.
Experience in resettlement from out-of-area placements, transitions to Adult Services, supporting independent living, and safeguarding investigations is desirable but not essential.
About You
You are passionate about supporting adults with learning disabilities and committed to improving their quality of life. You can build strong relationships with service users, carers, colleagues, and external partners, and are confident working in a hybrid environment.
You bring strong communication and decision-making skills, can manage and prioritise a varied caseload, and have a good understanding of current legislation, guidance, and best practice. You are resilient, adaptable, and comfortable working within a multi-disciplinary setting.
