We are seeking a dedicated, enthusiastic, patient-focused, and motivated clinical pharmacist to join our high-performing pharmacy team to manage the service provision to the Elderly Care & Frailty Clinical Service Line. The post holder will lead in all aspects of medicines optimisation for the Elderly Care & Frailty areas and will further develop our well-established role in medication transfer and the Discharge Medicines Service.

The post also involves supervising and role modeling for other pharmacists to lead, plan, and develop an advanced specialist clinical pharmacy service aligned with local and national policies. Responsibilities include ensuring the safe, clinical, and cost-effective use of medicines and staff resources, delivering key performance indicators, and working closely with other Lead Pharmacists.
The role includes delivering specialist pharmaceutical care, possibly including non-medical prescribing, managing a defined patient caseload, undertaking risk management, and ensuring compliance with clinical guidance. Support will be provided to the Principal Pharmacist in risk, financial management, and clinical governance. The post also involves leading clinical pharmacy audits, practice research, and supervising junior pharmacy staff.
