Social Worker – Transition Team (Adult Integrated Care)
It is an exciting time to join Adult Integrated Care, where we use a strengths‑based approach to promote independence and meet the health and wellbeing needs of vulnerable young people and their carers.
We are seeking an enthusiastic and hardworking Social Worker to join our Transition Team, who cover the Wakefield District.
About the Role
You will work closely with key partners in Children’s Services to carry out person‑centred assessments with young people aged 16–25 who are transitioning from Children’s services into Adult Social Care.
Using a strengths‑based approach to assessment and support planning, you will:
1. Promote independence and self‑determination
2. Focus on positive change and prevention of harm
3. Support young people to achieve their personal outcomes
4. Undertake holistic assessments early in the transition process to ensure a smooth and successful transfer into Adult Social Care
The Team
The Transition Team consists of 4.5 FTE Social Workers. You will be joining a motivated, supportive, and forward‑thinking team where supervision, development, and collaboration are priorities.
Working Arrangements
5. Monday to Thursday: 8:30am – 5:00pm
6. Friday: 8:30am – 4:30pm
7. Flexible working arrangements are available and can be discussed with the line manager

As a Social Worker, you will:
8. Be passionate about making a difference and open to change
9. Demonstrate the ability to work under pressure
10. Be well‑organised and able to prioritise and manage risk within a complex caseload
11. Have experience and a strong commitment to strengths‑based practice
12. Be able to undertake standard social care duties, including:
13. Chairing meetings
14. Providing support and advice to junior staff
15. Producing high‑quality, person‑led assessments and reports (including court reports)
A full driving licence is essential, as the role covers the whole Wakefield District and some young people may be placed out of area.

Newly Qualified Social Workers (ASYE)
All newly qualified Social Workers will undertake the Assessed and Supported Year in Employment (ASYE) programme. Wakefield is committed to supporting NQSWs, offering:
21. A dedicated ASYE assessor
22. Regular 1:1 supervision sessions
23. Training and team support
Your first year will be a probationary period, during which we will work closely with you to support your development and progression in your Social Work career.
Essential Requirements
Applicants must:
24. Hold a Degree in Social Work or equivalent
25. Be registered with Social Work England
26. Have relevant post‑qualifying training and experience
27. Hold a full driving licence
