Overview
Ceredigion County Council Targeted care & Enablement Team is recruiting. We welcome applicants who would like a career supporting individuals to regain skills and independence following acute illness or injury. This is an interesting and varied role involving activities of daily living such as personal care, mobility, meal preparation and medication in clients' own homes. We require 1 support worker to work split shifts on a 4 days on/4 days off rolling rota. The hours are 7:00–13:00 and 16:00–20:00 daily. The service is an Occupational Therapists–led setting with client-centered goals to regain as much independence as possible. The role is varied and you may cover long-term bridging while sourcing a package, further expanding your skills and knowledge. Clients have visits scheduled for specific times and you will manage your workload via an electronic call monitoring system to stay on schedule.

Our functions include: Porth Gofal Intake and Triage Team; Targeted Intervention Services; Fostering Services; Residential and Day Care Services; Housing Services; Integrated Community Equipment Stores; Emergency Duty Team. Responsibilities
* Provide support with activities of daily living (personal care, mobility, meal preparation and medication) in clients\' own homes.
* Work across a 4 days on/4 days off rolling rota with split shifts.
* Collaborate with Occupational Therapists to set client-centered goals and work to regain independence.
* Cover long-term bridging while awaiting package sourcing, expanding skills and knowledge.
* Adhere to scheduled visits and manage workload via an electronic call monitoring system.
Qualifications and Requirements
* Full driving licence, car and ability to travel within the County.
* All Wales Induction Framework (AWIF) for Health and Social Care to be completed within the first six months of employment for registration with Social Care Wales.
* Level 2 QCF in Health and Social Care or equivalent (to hold or achieve within a reasonable timescale).
* Welsh language level – Verbal Level 3; basic conversational Welsh with reasonable support to reach this level.
