An opportunity has arisen for a permanent full-time Social Worker in the Countywide Reviewing Team in Adult Social Care Mental Health. We support adults over 18 with severe and enduring mental illness, people living with dementia, and individuals accessing support for alcohol and substance use.
We apply a blended approach to our reviews through telephone, technology, and home visits to understand and review an individual’s interests, aspirations, concerns, and needs. Hybrid working arrangements are applicable, including elements of office-based work, client visits, and working from home. The successful candidate will need to be able to travel independently between locations, including out-of-county areas on occasions.
Responsibilities
* Support a variety of review activity, including post-hospital reviews, Section 117 Aftercare, reviews for those in receipt of a Direct Payment, and statutory annual reviews in individuals’ own homes (including supported living and care home settings).
* Work with partner agencies, carers and families to build networks of support to help people maintain their independence.
* Collaborate with the team to provide empowerment-focused care and to maximise individuals’ independence.
Qualifications
* A registered social worker – include your registration number on your application.
* Experience working with adults is essential.
* A good understanding of the Care Act 2014 legislation, Section 117 aftercare.
* An understanding of strength-based approach and prevention.
* Assessed & Supported Year in Employment completed.
