Role Overview
Registered Health Professional qualified in Physical Health Assessment and Non‑Medical Prescribing to join the Acute Visiting Service (AVS), part of Acute Care at Home across Stoke on Trent and Staffordshire Moorlands. The clinician will act within professional boundaries and competencies, providing care to patients in their own homes and reducing the risk of hospital admission. This role focuses on reducing ED attendances and safe admission avoidance, delivering acute/subacute interventions to improve outcomes.
Working Hours
Primary AVS shifts Monday‑Friday 13:00‑20:00, with additional support for patients all week 08:00‑22:00 as required.
Responsibilities
* Assess, diagnose and treat patients referred to the AVS during the referral hours 12:00‑20:00 Monday‑Friday.
* Conduct initial history taking, clinical assessment, diagnosis, treatment, referral (if required), evaluation and follow‑up of care.
* Collaborate with the Acute Care at Home Service and Virtual Ward Team to manage complex patients with acute needs.
* Provide support to the Community Rapid Intervention Service where appropriate.
* Develop and implement robust medical management plans that safely keep patients at home.
Qualifications
* Registered Health Professional with experience in urgent or primary care.
* Qualified in Physical Health Assessment and Non‑Medical Prescribing.
* Full driving licence and access to a vehicle for work.
