The Care Manager is responsible for coordinating the delivery of services to guests in collaboration with the facility’s team members.

Responsibilities
* Monitor and document the effectiveness of treatment provided.
* Coordinate the admission process and collaborate with the interdisciplinary team (IDT) to determine appropriate post-discharge services.
* Coordinate and update the Care Plan.
* Attend daily IDT meetings and provide clinical reviews as needed.
* Complete sections of Discharge Plan Instructions and review post-discharge instructions with guests upon discharge.
Qualifications
* Possess a Nursing Degree from an accredited institution or be a graduate of an approved LPN/LVN program.
* Hold a current, active, and unencumbered license to practice as an RN or LPN/LVN through the State Board of Nursing.
