The Enhanced Primary Mental Health Service (EPMHS) provides a multidisciplinary service to individuals with moderate and severe mental health difficulties in primary care. The focus is on psychological support for those who do not meet criteria for Adult Community Mental Health Services or NHS Talking Therapies Teams. We are seeking a social worker to develop this role, lead work with complex cases, support staff providing social care, and advise the wider MDT on social care matters.
As a Social Worker, you will be part of a multidisciplinary team, delivering high-quality social care to adults of all ages experiencing mental ill-health. Your core responsibilities include assessment, care planning, and interventions, including care coordination, from a social perspective. You will provide recovery-focused support to service users and their carers, emphasizing choice, control, and independence, while ensuring safety. You will champion social care and safeguarding within your locality.
The role involves assessing needs and outcomes, analyzing information related to service users and carers, and making professional recommendations. You will involve service users and carers to learn from their experiences. Additionally, you may be asked to conduct surveys and audits locally and contribute to trust-wide initiatives.
You will play a key role in social care assessments and commissioning packages of care, ensuring compliance with statutory functions as outlined in the Care Act 2014.
