Overview

We are looking for an exceptional Staff Nurse to join our dedicated multi-disciplinary team on our state of the art newly redeveloped Inpatient Unit providing assessment, symptom management and end of life care to our patients living with a life limiting condition. Length of stay can range from a few days to a few weeks. You will work as part of that team to provide holistic person-centered care, integrating nursing care with psychosocial, physical, rehabilitative and spiritual needs.

You need to be able to demonstrate evidence-based practice, up to date clinical skills, flexibility and a commitment to the delivery of high quality, effective and efficient nursing care. There is a comprehensive induction with individual mentor and opportunities for professional development. Palliative care/oncology experience desirable but not essential. This is a 25 hour contract worked over 2 shifts.


Main duties of the job

Key Responsibilities

* To continually develop clinical expertise and act as a positive role model promoting a high standard of care to fulfil the Nightingale House mission statement and aims
* To deliver evidence based care to patients and their carers in line with their physical, psychosocial and spiritual needs.
* To promote and maintain continuity of high standards of nursing care in the absence of the person with continuing responsibility.
* To be responsible following induction period for the assessment of care needs, the planning, delivery and evaluation of care; and to offer supervision and support to junior staff in the clinical area.
* Provide supervision and guidance to junior staff as appropriate and required within role and responsibility.
* To work at all times in line with the NMC code of conduct and duty of candour requirements.

Responsibilities (Clinical)

* Work as a member of the multi-disciplinary team to provide holistic person centred care, integrating nursing care with psychosocial and spiritual needs.
* Assist with initial and on-going assessment of patient and family needs and develop skills as a named nurse.
* Act as an associate nurse to plan, initiate, supervise and participate in patient and family care, discussing issues with senior team members as appropriate.
* Evaluate and update nursing care plans in response to changing problems/needs of patients and families.
* Monitor nursing practice, encourage development of colleagues through discussion of current and proposed care plans.
* Maintain appropriate and adequate records of your work.
* Maintain effective communication with patients and families throughout their contact with the hospice; provide telephone support and advice, access to nursing/medical staff for information, and bereavement support and follow-up as needed.


Management

* Take responsibility for the management of the inpatient unit team when required in the absence of more senior staff, following the Mission Statement and aims of Nightingale House Hospice.
* Liaise with multi-professional agencies to provide continuity of patient care.
* Maintain high standards of nursing care when in charge of the team and unit; foster positive relationships among all care stakeholders.
* Coordinate activities when in charge of the team, supported by the Ward Manager and Head of Clinical and Patient Services.
* Promote safety, well-being and the interests of patients, staff and visitors per hospice guidelines.
* Follow the Code of Professional Conduct to ensure compliance by all staff in your sphere of responsibility.


Education and Development

* Identify own learning objectives and attend relevant study/shadowing opportunities with line manager.
* Maintain records of supported study and provide evaluation for inclusion in personal file.
* Adhere to NMC revalidation requirements; participate in feedback and service improvement opportunities.
* Participate in personal development review (PDR) and objective setting at 6 month and subsequent reviews.
* Develop up-to-date knowledge of hospice policies and their impact on practitioners.
* Be aware of and disseminate current best practice knowledge as appropriate to role.
* Work with Practice Development Nurse to monitor effectiveness of services and areas of special interest.
* Participate or lead in service audits and provide feedback to colleagues.


Governance & Compliance

* Cooperate with the clinical governance framework; include risk management, service user participation, staff management, CPD, information management and clinical effectiveness & audit.


General Requirements

* Comply with Hospice terms and conditions, health professional guidelines and professional codes of conduct.
* Maintain competence; raise concerns with line manager if unsure of capability.
* Adhere to Hospice policies, procedures, protocols and guidelines.
