Responsibilities
* Provide expert care in patients’ homes and intermediate care placements.
* Manage your own caseload across varied and often complex conditions.
* Work closely with GPs and wider healthcare professionals to prevent hospital admissions and support early discharges.
* Participate in flexible weekend and late duty rotas.
* Cover community wards as required.
