Overview

To provide a safe and effective patient-focused specialist clinical pharmacy service to a specified patient group(s) supporting medicines governance and quality improvement activities within specialist area(s) to ensure safe, effective and patient-centred systems in place. This role focuses on Frailty and contributes to the seven-day Frailty Service within the unit, working as an integrated member of the consultant-led multidisciplinary team. There is a requirement to be an established practicing independent prescriber due to complexities of frailty, with prescribing and de-prescribing integral to the role.

New role development includes contributing to developing the clinical governance agenda and associated systems for medicines use within the service.


Responsibilities

* Provide pharmaceutical advice for staff working within frailty
* On a rota basis, provide clinical pharmacy cover over weekends and public holidays
* Undertake quality improvement initiatives and liaise with key stakeholders as appropriate
* Facilitate and support medicines communication at interfaces of care (e.g., discharge letters, urgent discharge processes)
* Liaise with health and social care services related to medicines (social work, acute hospital departments, General Practice, NHS Lanarkshire services, Community Pharmacy, and other stakeholders)
* Deliver pharmacotherapy activities (medicines reconciliation, education and quality improvement)
* Participate in multidisciplinary consultant-led ward rounds

There will be a focus on optimal use of medicines to ensure a patient-focused, efficient, progressive, resilient, safe and effective service to frailty.


Location and Organisation

Location: Pharmacy within University Hospital Wishaw. The working pattern for this role is 5/7.


What you'll need to apply

* Masters of Pharmacy Degree
* Registered member of General Pharmaceutical Council
* Independent prescribing qualification or be working towards
* Evidence of postgraduate training and professional development in clinical pharmacy practice (e.g., Post Registration Foundation Framework/1st year clinical diploma)
* Relevant post-registration experience in clinical pharmacy
* Ability to monitor patients’ therapeutic regimens to ensure effectiveness and reduce toxicity
* Ability to work effectively as part of a team in a pharmacy and multidisciplinary environment
* Commitment to revalidation as per GPhC directions
* Good verbal and written communication skills
* Self-motivation
* Participation in weekends and public holiday service
* Participation in pharmacy emergency duty service
* Willingness to contribute to the general functioning of the pharmacy department
* Full UK/EU/EEA driving licence (where a licence is required)

Desirable additional experience includes demonstrable clinical pharmacy service delivery to a defined patient group, patient information/education, supervising staff, knowledge of care planning, and project/practice research and audit skills. Ability to prioritise and organise workload.
