Occupational Therapist, Integrated Discharge Team

Our Integrated Discharge Team (IDT) is at the heart of smooth patient transitions from hospital to home or other care settings. We bring together the expertise of health and social care professionals to make sure each discharge is safe, timely, and tailored to the individual's needs.
As an Occupational Therapist, you will use your specialist skills to assess functional needs, plan discharge pathways, and coordinate support that promotes independence and dignity. You will work alongside a supportive, multi‑disciplinary team that includes nurses, physiotherapists, social workers, and community partners – all committed to seamless, patient‑centred care.
Main duties of the job
* Assess, plan and coordinate safe and timely discharges via the most appropriate discharge pathway to meet patients’ required needs.
* Manage a caseload of patients with varying diagnoses and/or complex needs, including physical, learning and mental health disabilities.
* Educate and train the team on the role of OT and provide day‑to‑day management for junior staff, ensuring supervision and annual review.
* Participate in planning, development and evaluation of the discharge team service within the acute trust and the wider integrated services.
* Take a lead for information management to ensure high‑quality data is recorded, evaluated and reported in a timely manner.
