Job Overview
Support Worker within the Care and Support Team. The purpose of the role is to provide autonomous assistance to the people we support to fulfil their potential for independence, choice, inclusion, and control.
You will be responsible for ensuring care and support is of a high standard, focused on person‑centred outcomes, and delivered within budget.
You will ensure that all needs assessments, support plans and risk assessments are kept up to date. There will also be direct engagement and liaison with social workers, family members and key workers as required.
Location & Travel
This role will be based at our Grange Village and Oaklands Park sites in Newnham‑on‑Severn. You must be able to travel to these locations and between them as part of your role.
Responsibilities
* Plan and provide support and care in accordance with individual person‑centred support and care plans, recording and reviewing progress so that the people we support can achieve independence, choice, inclusion and control.
* Maintain a clean, tidy and welcoming living environment.
* Use appropriate laundry equipment to enable laundry and clothing care.
* Plan, shop and prepare healthy meals considering health, choices and wishes.
* Assist in arranging and attending medical appointments.
* Budget, monitor finances and access benefits.
* Maintain an active social life in accordance with individual preferences.
* Provide personal care where required as part of care plans (e.g., bathing, hair washing) when registration permits.
* Administer and record medication, check and order medications.
* Plan menus, make shopping lists, prepare, cook, and safely store food.
* Handle related paperwork: support plans, daily updates, person profiles, risk assessments, incident reports, behaviour reports, and notes on health appointments.
* Carry out other tasks directed by the Team Leader to maintain high quality service.
* Work within community policies, procedures, and code of conduct promoting equality, diversity and human rights.
* Undertake training and continuous professional development per the Care Quality Commission, local authority requirements, and service needs.
* Work in partnership with family members and other professionals involved in individuals’ lives.
* Develop understanding of the Trust’s philosophy and values to inform practice.
* Sleep‑ins are required for this role.
Person Specification
Knowledge & Experience
* Experience supporting adults with learning disabilities, autism and distressed behaviours/emotions preferred but not essential – training provided.
* Good numeracy and literacy skills for completing official documentation to a professional standard.
* Driving licence desirable but not essential.
