Probus Surgery is a well-established Primary Care business offering NHS services to our local population, with a population of circa 9500 patients. Probus is our main surgery, with branch sites at The Merlin Centre and Tregony. Our clinical team consists of GPs, Advanced Clinical Practitioners, Nurse Practitioners, Paramedics, Nurses, HCAs, Pharmacists, Dispensers, Care Co-Ordinators, Social Prescribers, Receptionists and Admin.

Job responsibilities
Please see attached Job Description.Thefollowing are the core responsibilities of a Practice Nurse. There may be occasions when there is arequirement to carry out other tasks; this will be dependent on factors such asworkload, staffing levels and direction of the Clinical Team or Senior Manager.
* Independentlyassess, diagnose, treat, refer, signpost and discharge patients and serviceusers as necessary and in accordance with clinical based evidence and NICEguidelines
* Undertaketelephone triage
* Undertake the collection of pathology specimens, including bloodsamples, swabs, etc.
* Undertake simple wound and complex dressings, inclusive of legulcers
* Undertake smears
* Undertake contraception/pill checks and support our woman healthclinics
* Perform investigatory procedures as requested by colleagues
* Use appropriate infection control procedures
* Be aware of safe storage of equipment, and drugs
* Ensure clinical treatment is safe, effective and conductedwithin the boundaries of competence, and to acknowledge limitations
* Ensurepersonal and professional clinical standards are maintained
* Adaptstyles of communication in order to empower self-management and healtheducation for patients
* Participatein vaccination programmes, vaccinating patients in the Practice, and/or on homevisits
* Formally and informally, impart knowledge and skills tocolleagues, promoting peer review and best practice within the workingenvironment
* Communicate at all levels within the team, ensuring an effectiveservice is delivered
* Work within Practice policies and procedures
* Enhance own performance through continuous professionaldevelopment, imparting own knowledge and behaviours to meet the needs of theservice
* Contribute positively to the effectiveness and efficiency of theteam, and work colleagues
* Establish and maintain good liaison with other Practices, andthe wider system, including secondary care
* Support the clinical team with safeguarding matters, inaccordance with local and national policies and refer safeguarding concerns,using established safeguarding referral pathways
* Delegate tasks clearly and appropriately, adopting theprincipals of safe practice and assessment of competence of those colleaguestaking on the delegated duty
* Ensureaccurate and legible notes, of all consultations and treatment, are recorded inthe patients notes
* Ensurethe clinical system is kept up to date with accurate coding, using templateswhere appropriate
* Ensurethe appropriate items of service claims are made accurately, reporting issuesto the relevant person
* Collectand maintain statistical information, required for regular and ad hoc reportsand audits
* Attendand participate in Practice meetings, as required
* Undertakeany other additional duties appropriate to the post as requested by thePartners or Managers
Person Specification
Qualifications
* Registered Nurse with NMC
* Experience of managing a team
* Able to provide a culturally sensitive service, by supporting people from all backgrounds and communities, respecting lifestyles and diversity
* Ability to cope with occasional exposure to aggressive behaviour
* Exemplary employment record
* Hold a clean driving licence, with a fully insured car for business purposes
* Ability to communicate effectively, both verbally and in writing, with patients, their families and carers
* Have a strong awareness and understanding of when it is appropriate or necessary to escalate issues to clinical team/line manager
* Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines
* Ability to work flexibly and enthusiastically within a team or on own initiative
* Knowledge of, and ability to work to, policies and procedures, including confidentiality, safeguarding, information governance, and health and safety
* Experience of involvement in innovation or change
