We are offering an exciting opportunity within South East Essex Community Health Services to join the inpatient Intermediate Care Frailty/Stroke facility at Rochford Hospital.

Essex University Partnership NHS Foundation Trust is seeking talented and caring Healthcare Assistants to join our inpatient team.
Your role will involve assisting the senior team in providing care and physical rehabilitation to patients requiring a short-term recovery period outside their homes, following an acute event or deterioration, which may or may not have resulted in a hospital stay. The goal is to deliver person-centered rehabilitation to help patients achieve their maximum potential and live as independently as possible.

Main duties of the job
* Encourage patients to participate in daily activities and therapeutic exercises.
* Assist patients in achieving rehabilitation goals to improve independence and wellbeing.
* Participate in manual handling of patients as per the Trust’s Manual Handling Policy.
* Follow infection control policies and wear protective clothing as required.
* Observe, record, and report changes in patient health and environment to senior staff.
* Be familiar with emergency procedures and respond appropriately.
* Complete observations such as urinalysis and blood sugar monitoring.
* Collect patient specimens as needed.
* Record dietary needs and fluid intake.
* Assist the senior team with visits and case conferences.
* Work collaboratively with therapists to support assessment and treatment.
* Contribute to care planning and risk management.
