The Cancer Services Team is seeking Cancer Follow-up Coordinators to work alongside Cancer Support Workers and Clinical Nurse Specialists, as well as the wider Cancer team. The role involves ensuring patients have a timely, efficient, and supported follow-up after completing their cancer treatment. Post-holders will be based within a specific cancer-type team and collaborate closely with both clinical and non-clinical team members.
These roles are available across all hospital sites: Royal Sussex County Hospital, Worthing Hospital, and St Richard's Hospital.
Key Responsibilities:
1. Track patients post-treatment, including enrolment and discharge from remote follow-up systems.
2. Coordinate investigations and symptom assessment questionnaires according to a predefined schedule.
3. Evaluate results within a clinically supervised protocol.
4. Communicate review outcomes to patients via standardised letters.
5. Escalate issues to the clinical team as indicated by the protocol.
