An exciting opportunity has arisen for a band 4 Lead Senior Health & Social Care Worker to join the Discharge to Assess Service Nairn & Ardersier on a permanent basis. You will be expected to work flexibly to meet the needs of the service which will mean rotation of shifts to ensure service provision over 7 days.
This role requires high levels of professionalism, to work effectively across complex multi-agency environments, where partnership working is essential. Excellent communication skills, collaborative working and highly developed interpersonal skills are required.
The successful candidate will support the caseload holder to deliver and assess patient centred care. You will work in close partnership with colleagues, ensuring an integrated approach to the delivery of compassionate care is met.
A requirement is to be registered SSSC and have a driving licence with access to a car.
