We are seeking a motivated Occupational Therapist to join our Locality Therapy Team. The successful individual will be based at Ludlow Hospital, working on the Community Hospital ward. They will join an enthusiastic and supportive multi-disciplinary team in providing high quality care, rehabilitation and discharge planning.
The successful individual must have an interest and experience in patient rehabilitation within the Community setting and be able to work as an autonomous practitioner. The post holders’ main responsibilities will be providing Occupational Therapy assessment and rehabilitation to patients who have a wide range of conditions and complex needs.
Main duties of the job
* To provide a high quality seamless Occupational Therapy service from Acute or Community Hospital to Community, responsive to the needs of the local population as part of a cohesive multidisciplinary team.
* To work as a key member of the multidisciplinary team (MDT) that is responsive to the needs of the local population, working flexibly to respond to the changing needs of the patient.
* To provide a key role in facilitating the seamless transition from community hospital to patient’s own home and transferring the care of the patient to other appropriate services to maximise the patient's independence and quality of life.
* To provide supervision, facilitate the personal and professional development of junior members of staff, enhancing their skills and knowledge and where appropriate line management.
Qualifications and Training
* Diploma/degree in Occupational Therapy
* Current registration with HCPC
* Evidence of core rotations
* Evidence of specialist post graduate training and education with older adults.
Experience
* Significant Post graduate experience in a variety of clinical settings with experience in a community setting and inpatient setting
* Experience of working in multi-disciplinary / multi-agency teams.
Skills and Knowledge
* Ongoing awareness of recent developments in practice and research including NSF’s/NICE guidelines.
* Understanding of clinical governance and risk management and their implications for the service.
* Excellent interpersonal and communication skills.
Other
* Must be able to meet travel requirements of the post.
