What you will do:
* Coordinate care for complex cases under the Care Programme Approach (CPA) and create co-produced care plans with service users
* Conduct comprehensive risk assessments and manage potential risks to ensure client safety and wellbeing
* Deliver holistic assessments of mental health and social care needs, including Care Act assessments
* Work collaboratively with service users, carers, and support networks, alongside statutory and voluntary organisations
* Partner with inpatient units to support seamless service provision and effective discharge planning
* Maintain accurate, timely, and comprehensive records and reports in line with professional standards
* Apply specialist knowledge to engage with diverse communities and uphold safeguarding responsibilities and legislation, including the Mental Health Act and Mental Capacity Act
