We are looking for a motivated and skilled Care Coordinator to join our Home Care team within Staffordshire County Council’s Provider Services. Our mission is to deliver high‑quality, person‑centred care that enables adults with a wide range of needs to thrive in their communities.

As a Care Coordinator, you will play a vital role in the day‑to‑day running of our service—overseeing staff, coordinating care delivery, and ensuring the people we support receive safe, effective and personalised care. You will lead by example, supporting and supervising remote teams, managing referrals, and working closely with families and professionals to achieve the best possible outcomes.

The Care Coordinator will work within a fast‑paced and dynamic environment, managing a high volume of daily referrals received from Social Work teams. As our service provides short‑term reablement support for up to a maximum of six weeks, there is a continual flow of new referrals and customers.

The successful candidate will be responsible for planning and maintaining effective staff rotas, using creativity and problem‑solving skills to maximise capacity and ensure the delivery of approximately 400 care hours each week. This role requires strong organisational abilities, the confidence to prioritise competing demands, and a proactive approach to meeting service targets.

This role requires travelling within the community, primarily Stafford/Burton, and so a full UK driving licence and access to your own vehicle is essential.

In this role you'll:

* Develop, implement and review person‑centred care plans and risk assessments for people referred to the service.
* Coordinate staff scheduling to ensure safe, efficient delivery of 400 care and reablement support hours in the community
* Manage referrals from social work teams and assess service needs, creatively planning rotas to maximise care‑hour capacity and match staff effectively to demand.
* Provide day‑to‑day leadership, supervision and support to remote staff, including managing rotas, leave and training.
* Use systems and technology to monitor quality, improve efficiency and maintain accurate records.
* Ensure compliance with safeguarding, MCA, Care Act, health and safety and CQC requirements.
* Oversee medication safety, including auditing, reporting issues and ensuring staff adherence to policy.
* Participate in the on‑call rota and provide flexible operational support as required.
* Travel across the county and work flexibly (including some evenings/weekends) to meet service needs.

What you will have:

* Experience working within an adult social care provider setting.
* Previous experience supervising or supporting staff, including performance and practice oversight.
* Experience of rostering 400 or more care hours a week
* Knowledge of person‑centred care, reablement, strengths‑based approaches and active support models.
* Understanding of care planning, reviews, risk assessments and positive risk‑taking.
* Knowledge of key legislation including safeguarding, MCA, Liberty Protection Safeguards and the Care Act.
* Experience with medication administration and safe handling procedures.
* Good awareness of health and safety requirements in home care settings.
* Strong IT skills and the ability to maintain accurate electronic records.

Ability to travel independently and work flexibly across shifts, including participation in an on‑call rota.
