Job Title: Occupational Therapist Band: 6 Location: Prescot, Lancashire Working Hours: 37.5 hours per week Rate: Competitive — details to be discussed upon application We are seeking a proactive and compassionate Band 6 Occupational Therapist to join our NHS client’s Frailty Response and Acute Care Team in the community setting in Prescot, Lancashire. This is a rewarding opportunity to support older adults and individuals with complex needs, helping to prevent hospital admissions and promote independence at home. Key Responsibilities Deliver high-quality occupational therapy assessments and interventions in patients’ homes and community settings. Work as part of a multidisciplinary frailty response team to provide rapid support to individuals experiencing acute health deterioration. Develop and implement person-centred care plans aimed at maintaining function, safety, and independence. Support discharge planning and admission avoidance through timely and effective interventions. Liaise with families, carers, and other health and social care professionals to ensure coordinated care. Contribute to service development, audits, and quality improvement initiatives. Provide supervision and support to junior staff and students as required. Qualifications & Experience Degree or diploma in Occupational Therapy HCPC registration as an Occupational Therapist Experience working in acute care, frailty, or community settings Strong clinical reasoning, communication, and organisational skills Ability to work autonomously and as part of a multidisciplinary team UK driving licence and access to a vehicle for community visits (desirable) How to Apply If your experience aligns with this role, apply today!
