Join our community heart failure service team as a Patient Service Coordinator, where you will play a vital role in enhancing patient care and supporting our multidisciplinary team.
As a Patient Service Coordinator in the Community Heart Failure Service, you will be a central figure in enabling seamless, patient-centred care for individuals dealing with heart failure. This non-clinical role is essential for enhancing access, reducing clinician burden, improving patient experience, and supporting service efficiency and quality.
This post is part time 30 hours per week.
The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.
Coordinate follow-ups, obtain diagnostic reports, and handle referrals, improving continuity of care and assist the clinical team in preventing readmissions or deterioration.
Manage timely and appropriate patient bookings, reducing delays in care and ensuring patients are seen at the right time by the right clinician.

Induction and Start Date
All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction / initial training requirements of the role.
