Join to apply for the Advanced Clinical Practitioner role at Hywel Dda University Health Board.

St Caradog Adult Mental Health Inpatient Ward are looking to welcome an Advanced Clinical Practitioner to their Multi Disciplinary Team.

This Band 8b post is open to applicants who have Approved Clinician Status or open as a development Band 8b Annex 21 post to a qualified Advanced Practitioner who has a willingness and commitment to achieve Approved Clinician Status within an agreed timeframe. The successful development candidate will be appointed under the All Wales Annex 21 rules. The post holder will hold an independent prescribing qualification and advanced specialist knowledge of Mental Health patient pathways is essential. A development plan will support progression to the Band 8b role, with staged payments during the initial years and supervision to develop required competencies.

As part of the Annex 21 process, there is no financial penalty to those who apply for this role where the percentage will cause a reduction in pay; a local protection would be put in place until pay has caught up.


Responsibilities

* Provide clinical assessment and direct the management of patients.
* Be autonomous in decision making, including judgments involving highly complex facts/situations requiring analysis, interpretation and comparison of options in relation to care/management needs on a day-to-day basis.
* Identify need for change, lead innovation and manage change including service development, developing a case for change, and negotiating and influencing change.
* Carry out complex patient clinical reviews and feedback complex clinical information to enable decisions in care management, which may involve differences of opinion.
* Lead and chair ward reviews, including initiating and titrating treatment plans.
* Prepare medical reports.
* Support doctors’ development by arranging opportunities in other clinical areas for them.
* Review formal patients and work in collaboration and joint decision making with RCs regarding ongoing detention and least restrictive principles.
* Provide clinical assessment using advanced knowledge and clinical skills to develop diagnoses and management plans for optimal care.
* Provide Advanced Clinical Practitioner Strategic Service clinical leadership.
* Responsible for policy and service development.


Qualifications and requirements

* Independent prescribing qualification.
* Advanced specialist knowledge of Mental Health patient pathways.
* Band 8b eligibility: Approved Clinician Status or Annex 21 development pathway with commitment to achieve Approved Clinician Status within an agreed timeframe.
* Willingness and ability to work within the All Wales Annex 21 framework and to participate in a development plan with supervision and support.
