Job Overview
Stockport Learning Disability team is a truly multidisciplinary team working with individuals with a learning disability from the age of 18. The team delivers high quality health care assessments and interventions to adults with complex and challenging needs and is based at Stopford House, serving the Stockport area.
Key Responsibilities
* Deliver high‑quality health care assessments and interventions to adults with learning disabilities who have complex and challenging needs.
* Work within a multi‑disciplinary team that includes community nurses, speech and language therapists, social workers, psychologists, assistant practitioners and psychiatric input.
* Provide person‑centred, creative and innovative support to individuals, families and carers.
* Undertake a range of comprehensive assessments for individuals with learning disabilities and complex needs.
* Monitor and evaluate complex interventions and care packages.
* Offer behaviour support training to identified colleagues and teams, including the development of specialised care plans.
* Coordinate clinical care within a defined caseload, accessing nursing supervision when appropriate.
* Promote public health, well‑being and prevent ill‑health in the community setting.
* Identify and manage risk, including participation in safeguarding adult investigations.
* Collaborate with primary and secondary care services to ensure health needs are met.
* Represent individuals as unique and valid citizens, supporting inclusion.
* Prioritise competing demands and manage caseload effectively.
* Support people with learning disabilities who may exhibit challenging behaviours, including verbal and/or physical aggression.
