Offer description

We are currently looking for a full‑time Health Care Assistant who will work as a member of the Wisbech District Nursing Community Team undertaking delegated duties to meet the needs of service users. You will be trained and supervised by registered professionals and will be expected to follow a plan of care based on the specific needs of the service users.

This is a great opportunity for an experienced, motivated and enthusiastic individual to join our service providing care in patients’ own homes.


Qualifications

* Experience of working in the community.
* Experience of working within health or social care.
* Experience of caring for and supporting adults and older people.
* Demonstrated commitment to high quality care and service provision.
* Good communication skills.
* The ability to work flexibly and as part of a team, as well as individually.


Job Responsibilities

* Make judgements on patient response to prescribed treatment/care plans and report findings to a qualified professional in line with the competency framework.
* Overcome barriers to understanding, e.g., patients with hearing or cognitive impairment.
* Communicate with patients and carers by exchanging factual information, reassurance, tact and empathy.
* Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
* Be responsible for ensuring equipment is used safely, following relevant training, and that it is maintained appropriately.
* Work under guidance on specified tasks, taking direction from registered and non‑registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner.
* Demonstrate an awareness and understanding of consent and gain consent as appropriate following organisational policies.
* Attend relevant external and internal courses to extend knowledge or gain relevant skills to improve clinical practice as identified in the Personal Development Review (PDR).
* Be exposed to bodily fluids, infected material, blood products; therefore must utilise universal precautions and adhere to infection control policies.
* Attend meetings as required.
* Ensure patients personal care is provided and highlight areas of concern such as pressure areas and report any changes to professional.
* Work within the wider disciplinary team and recognise own role within it.
* Promote patient independence and self‑management.
* Provide and maintain a safe, comfortable and dignified environment for all patients/clients and carers.
* Practice the principles and philosophy of individualised patient care in accordance with Trust Policy, procedures and regulatory body guidelines.
* Maintain appropriate and timely documentation of each patient contact (ensuring these are recorded on SystmOne or RIO within 24 hours).
* Plan and prioritise own visits to delegated patients, ensuring these are documented on SystmOne.
* Ensure patients personal care is provided, as appropriate.
* Any data that is taken/shared as part of a phone call or transported, faxed or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.
* Assist in the implementation of audit processes under direction from registered staff.

You must hold a full UK driving licence and have access to a vehicle, as travel is required around the county and use of public transport is not permitted.

You may be required to undertake and successfully complete the new national Care Certificate within a 12‑week timeframe if you meet our criteria. Full training and support will be provided. The Care Certificate sets out 15 national care standards that must be achieved by all health and social care staff to demonstrate the skills and knowledge needed to provide safe and excellent quality of care.
