Promote effective patient flow based on individual service users' care pathways, ensuring least restrictive practice, optimizing lengths of stay, and supporting the GMMH Trust-wide goal to reduce out-of-area placements.
Key Responsibilities:
1. Facilitate timely admissions and discharges within inpatient units through safe, effective bed management as part of the CPA process.
2. Act as a member of the Inpatient Multi-disciplinary team, providing clinical expertise to facilitate safe discharges.
3. Collaborate with agencies such as RAID, CMHT, HBTT, EIT, Later Life, LD, and Rehabilitation Services to maximize safe and timely discharges.
4. Provide education, advice, and supervision related to admissions and discharge planning, including training for junior staff and students.
5. Participate in Trust-wide Patient Flow meetings and cover Hub & Spoke teams as needed, including chairing bed management meetings and maintaining records.
6. Participate in local Bronze on-call rotas as required.
7. Ensure adherence to bed management policies, criteria, and legislation, including safeguarding and risk management.
8. Manage referrals, ensuring least restrictive options, completing documentation, and updating records.
9. Coordinate transport arrangements for transfers.
10. Maintain knowledge of relevant legislation, CPA procedures, and safety protocols.
11. Ensure KPI, CQUIN, and CQC targets are met and maintained.
12. Conduct clinical assessments and risk assessments to promote patient safety.
13. Maintain accurate clinical records and input data into information systems.
14. Engage in supervision, appraisal, training, and team meetings.
15. Participate in daily huddles to communicate demand and capacity.
